ASSESSMENT B – OBSERVATION REPORT (LEARNER)

LEARNER INSTRUCTIONS

You are required to do the following.

OVERVIEW OF TASKS

You must complete the following assessment tasks.

  • Use predetermined health, safety and security procedures and safe work practices in work functions on at least three occasions.
    • Respond to one emergency or potential emergency situation in accordance with organisational procedures.
    • Participate in one work health and safety (WHS) consultation activity. The consultation activity may be a staff meeting, discussion with a supervisor, manager or WHS representative or a formal report to a WHS representative.

ASSESSOR TASKS

Your assessor will do the following.

  • Provide you with an observation checklist that lists the skills/tasks they will observe you performing when completing specific tasks. Please familiarise yourself with the observable skills outlined in the task’s Observation report prior to starting an observable task.
    • Contextualise observations to suit your workplace or training environment.
    • Observe you in an operational (live or simulated) environment using a range of skills/tasks.
    • Ask you questions relating to specific tasks you are demonstrating to ensure you have the required knowledge.
    • Provide access to industry current equipment, technology and software, and legislative and regulatory resources and documentation as specified in the assessment conditions.
    • Use the checklist to observe you using a range of skills/tasks.
    • Record comments and feedback based on their observation of your performance.
    • Ensure that you can consistently perform all tasks multiple times (or where not specified, at least once) satisfactorily.
    • When necessary, complete an additional checklist as a record of the skills and techniques you are required to perform to achieve a satisfactory outcome in this assessment.

Task 3: Emergency procedures demonstration

  • In consultation with your team leader/supervisor, manager and/or health and safety representative, hold an emergency evacuation drill for your workplace or training area.

Description of incident/accident

During the emergency evacuation drill, an employee, Employee, tripped over boxes that were improperly stored near the emergency exit. As a result, they injured their left ankle. The incident occurred as we were evacuating the building, following the sound of the emergency alarm. Immediately after the incident, I sought assistance from our workplace first-aider, First-Aider, who promptly responded and administered first aid. The first-aider treated the employee’s ankle with ice and recommended further medical assessment. The boxes causing the obstruction were part of the office supplies and had not been stored correctly. This incident highlights the importance of keeping emergency exit routes clear at all times.

Scenario

It’s been two weeks now and it still hasn’t been dealt with. Each day it is turned on, the cord is sparking up more and more. Some of the employees are refraining from using it altogether, which is causing a hold-up for the customers.
