This assessment is to be undertaken in a fully equipped aged care facility.
For this assessment, you are to be observed on 3 separate occasions (2 observations by your
workplace supervisor and 1 observation by your assessor). For this activity you are required to
demonstrate your ability to provide support to 2 different people living with dementia.
You must ensure that you provide evidence of your ability to provide support to people with
dementia including:
a. Using person-centred care approaches with clients
b. Understanding and referring to the client’s care plan when necessary
c. Creating and maintaining a suitable environment for clients with dementia
d. Report to supervisor as necessary
e. Use different communication strategies in different situations
f. Use therapies such as validation therapy, reality orientation and reminiscence therapy
g. Organise activities for clients that reflect normal life
h. Consider clients’ individual likes and preferences as well as cultural aspects when
arranging activities
i. Maintain a safe environment for a person living with dementia
j. Liaise with, and provided support to family and carers
k. Identify behaviours of concern and their triggers
l. Partake in appropriate team collaboration in regards to client care
m. Formulate strategies to ensure minimal disruption to clients and the others around them

  1. Mavis suffers from severe dementia
    Your client Mavis suffers from severe dementia. Prior to her diagnosis, she participated in
    many social activities including the country women’s group, line dancing, and regularly
    attending church. But after the diagnosis, her condition worsened to the point that she could no
    longer live in her own home and she now lives in an assisted living facility. She regularly
    forgets what day it is and gets frustrated over her unfamiliarity with time.
    a. In what ways could you assist to make the transition from home to the assisted living
    facility easier for Mavis?
    First of all psychologically i could assist Mavis to make the transition from home to the assisted
    living facility easier. Then, i could stay focused on the feeling she were demonstrating and respond
    with verbal and physical expression of comfort, support, and reassurance. At the last but not the least
    i could assist her by caring, loving, spending time with her,using some practical strategies for dealing
    with her and setting a positive mood for interaction.
    b. How can you find out more information about Mavis?
    I can find more information about Mavis from her regular checkup doctor who is helping her for
    long term. I think it is the best way to get more information about her.

    c. Give two examples of how you would ensure person-centred practice in this scenario.
    In this scenario, I would ensure person-centred practice by getting the persons attention . For
    example; If she is sitting down then I will also get down to her level and maintain eye contact with
    her and keep smile. Before speaking, I would make sure I have her attention ; by identify myself by
    name and relation and I would use nonverbal cues and touch to help keep her focused. And second
    one is ;I would be patient in waiting for my loved ones reply and i would use some practical
    activities to convience her.
    d. When communicating with Mavis, what are some communication strategies you can use
    in order to gain her trust
    I can use some common languages like facial expressions,body languages to communicate our
    feelings more than words do. And i can touch her to help convey our message and show our
    emotions or feelings of affection.
    Some communication strategies like i can ask some simple questions and simple sentence at one
    time;those with yes or no answer work best.
    For example, “Would you like to wear red dress or blue dress?” Better still, I will show her the
    choices- visual prompts and cues also help her to clarify the question and can guide her response.
    e. Describe what activities you could arrange for Mavis in order to enhance her selfesteem as well as allow her to maintain as much independence as possible.

    I could motivate and encourage her by doing simple things to do what she can gently remind like she
    used to dance ,I will dance for her doing steps and using visual videos ,cues etc.
    I will keep her maximum funtional figuring out ways to do that probably decreases agitation and she
    will gets benefit doing these things; for eaxmple: sometimes she will not able to wear her dress at
    that time, I will do for myself wearing a dress for me then she can remember it and she will do
    herself . Likewise, I will use some utensils for food by using fork or spoon then pick up some food
    then its almost comeback to her and she remember how to do it even though it may have trouble to
    begin her but once initiate it it may be useful for her.Doing these things she will get pleasure from it
    and rewarded from it or feel like she is more independent.
    I could build full positive mood for interaction to communicate with her easily by showing visual
    prompts like body languages and doing funny expresssions like wise etc. I will keep patience in
    waiting for her reply. Doing these activities,she will be enhance her self-esteem as well as much
    independence.
    f. In what ways could you address any possible stigma in relation to this scenario?
    As every person is different it is important to acknowledge and support them as individuals -not
    just a person with dementia.
    In this scenario, I wont let her feel alone whatever the situation or condition is, I will be always their
    for her to assist her it may help her to address easily and I may need to try a number of different
    approaches before being able to provide assistance to her then she will get pleasure from it
    I could address her something different ways ,I will ensure her you are gentle, patient and beautiful
    as usual and I will give attention to her and ask her for a walk or for a dinner by showing visual
    vedios ,cues and using practical strategies, it will connect our feelings or emotions. Communication
    is the best trick to feel her free from stress, and spending lot of times with her by caring, loving, etc
  2. Beth goes for a check up
    Beth is your 67-year-old client who you are taking to the GP for a medical check-up. You arrive
    at the house to find her in the backyard arguing and hitting her husband with her hand and
    then with a newspaper. She is accusing him of having a lady friend staying in the house. She
    appears confused and aimlessly wandering in the garden, incessantly moving pot plants
    around and accusing the next door neighbour of stealing them.
    a. List the behaviours of concern exhibited by Beth.
    The behaviours exhibited by Beth are listed below:
    Poor judgement and confusion,Trouble handling herself, using unsual words to her husband,
    hallucinating,losing balance and problems with movement, acting impulsively, wandering and
    getting lost in a familiar neighborhood, arrogance and doing hauntiness behaviour etc.

There are many behaviours exhibited by Beth but the risk is hitting like hauntiness whatever she gets
she hits by them is very risk to me and arguing with her neighbours using unusual words is like
hallucinating.
For others personal safety I would give attention to her and patiently behave with her it may be
useful to divert her mind at that condition. At that situation , I will set positive mind for interaction
by speaking to her one in a pleasant and respectful manner.

Generally people with these behaviours become agitated due to three potential triggers: Medical,
physiological and environmental. Medical triggers can include sickness, fever, or pain but emotional
triggers, also called mental health or psychological triggers these behaviour may be a direct result of
changes in the brain. Environmental triggers can include dust mites and mold, pets ,cigarette and
other cigar smoke, viral or sinus infection, weather changes, pollution and exercise.
New or unfamilier caregivers or separation from love ones could also possible triggers for these
behaviours.
Triggers are anything that might cause a person to recall a traumatic experience they have had.
For example, graphic images of voilence might be a trigger for some people. These could be the
potential triggers for these behaviours.
e. In what ways could the other members of your team be of value when caring for clients
with dementia?
When a person is diagnosed with dementia ,it affects the whole family. I think taking a team
approach to dementia care is the best way to support clients and the families too.
The other members of my team care a patient with dementia, they offer education, respite care,
and other kinds of support to the clients. They encourage the clients by stimulating and
enjoying them ,make dressing and grooming easier, and help the person stay calm and
oriented.
f. In what ways could you provide support to Beth’s husband in this instance?
In this instance,I could support him first of all by psychlogically.
Then I will suggest him ,stay focused on her feeling what is she demonstrating and use verbal and
physical expression to respond her answer, deal with caring , loving and communicate patiently and
respond simple and clear answer to her.
I could also suggest him deal anything with patience and wait for answers. Use common languages
to communicate with her than words do and get attention of her. For example : you will offer her for
a walk and dinner, I think it will connect your emotioms and feelings.
She will feel happy and stressless by showing care , by paying attention for her, and speak with her
respetful manner, if she doesnot understand the first time , repeat again the same words and wait for
her answer and make the positive set of mind of her and yours and maintain your sense of humor .

A energy is focused on the patient ,and the family members feel stressful.
Although, caring of her, puts a huge strain on the family members it could be the possible
impacts on other family members.
Beth’s often experience changes her emotional responses, that may have less control over her
feelings and how to express her feelings to her family that strain could impacts on Beth’s illness.

    of dementia.
    Lately, Peter has noticed that he has not been receiving his mail as he did when he lived by

    This is an open book written test. Therefore, you are allowed to bring books/ resources that
    can help you to answer the test.
    Your assessor will set a date and time for this open book written test.
    On the test date:
